Good morning team!

Quick hits for today:

  1. High-Alert Med Focus
    Epinephrine auto-injectors (EpiPens) – we had two near-miss look-alike/sound-alike incidents last week with epinephrine 1:10,000 prefilled syringes. Double-check concentration and concentration labeling every time you pull or dispense. If you see a 1:1,000 vial being used for cardiac arrest, stop and verify with a second pharmacist.

  2. Stock Update

    • EpiPens (0.3 mg & 0.15 mg): Currently low on both pediatric sizes—only 8 adult and 5 junior left in main pharmacy. Central supply expects delivery by 1400 today.
    • Crash cart epinephrine (1 mg/10 mL) syringes: fully stocked.
    • “Dirty Epi Drip” bags (1 mg in 1 L NS) pre-made: 4 available in the code cart area—check expiration (good through end of shift).
  3. Process Reminder
    Anaphylaxis protocol refresh: IM into anterolateral thigh only—no more deltoid or gluteal for adults/older kids. Repeat dose q5–15 min if no response. Document exact time, site, and dose clearly in the MAR. If patient is less than 15 kg, confirm weight-based 0.01 mg/kg dosing and use junior EpiPen if available.

  4. Safety Moment
    Extravasation risk with IV push epinephrine: If you’re running a code or push-dose epi, use a large-bore peripheral line or central line when possible. Have phentolamine ready in the antidote box (5–10 mg in 10–15 mL NS for local infiltration if extravasation occurs). Report any infiltration immediately.

  5. Shout-Outs
    Big thanks to Sarah and Mike for catching the mismatched epinephrine vials during yesterday’s restock—great catch that prevented a potential error.
    Also, props to the night shift for prepping four extra Dirty Epi Drips—super helpful during the 0300 code.
